CASE SUMMARY: Ectodermal Dysplasia

Patient “Ryan” was referred to me by another dental professional in the area for a consultation of an All-on-X procedure. Ryan was diagnosed with Ectodermal Dysplasia, and his father had the same condition. Ectodermal dysplasia is a rare group of inherited disorders characterized by aplasia or dysplasia of tissues of ectodermal origin, such as hair, nails, teeth and skin. Dental manifestations include hypodontia, complete anodontia, or malformed teeth. At 26 years old, Ryan had already been wearing partial dentures since he was 18. He was not happy with his smile and was hoping to do dental implants

CASE SUMMARY: What do you do when your patient hits rock bottom?

Patient “Patricia” was referred to me by a local oral surgeon to evaluate a possible All on-4 dental implant treatment. Patricia already had a long history of dental treatment, including multiple implants and crowns, and she was getting tired of on-going treatment. Previous panoramic radiographs showed bone loss on multiple sites, and that the mandible was at very high risk of fracturing due to severe bone loss. Patient liability became an issue and she was sent to a local university for complete care along with removal of implants and possible mandibular stabilization. Three years passed and Patricia came back to our office to see if I could make any improvement to her condition. A new panoramic was taken, and this became one of the most challenging cases I have ever had to perform.

Patient “Angelo” was referred by a local dentist for a comprehensive exam and any possible treatment options. Angelo was seen by others as well. Most doctors had suggested starting with periodontal treatment or possibly sequential extractions prior to any removable prothesis and ultimately doing an implant treatments such as All-on-X. However, Angelo did not want to do any removable prosthesis nor any sequential treatment, so he was referred to us because we are usually able to do an All-on-Six treatment with a same day prosthesis.

CASE SUMMARY: Immediate Vs. Delayed Implant Treatment

Patient “Jorge” presented with a loose existing implant on #8, which had a draining fistula. The periapical radiograph shows severe bone loss and possibly a vertical fracture. He saw several dentists and his case was either rejected or the surgeon proposed a conventional method to resolve the issue, such as an extraction, bone graft, and implant placement at a later time after healing. Jorge didn’t want to wear an interim partial for 6-8 months until it was complete.

When restoring a complete arch with an implant-retained full arch prosthesis, the choice of material becomes extremely important. Acrylic surfaces on a titanium bar have been utilized for many decades now and have a high incidence of tooth fracture, veneer delamination, and occlusal surface wear. Zirconia frameworks with layering porcelain also have shown to have problems, including porcelain chipping or debonding of the titanium inserts. In this newsletter we’ll look at research comparing zirconia hybrid vs. acrylic hybrid, specifically addressing complications.
